Physician Documentation Initiative: Clinical Documentation for Risk Adjustment (HCC and RAF Fundamentals)

Structured as a guided clinical workshop, the activity moves from foundational risk-adjustment principles into condition-specific documentation and hands-on knowledge checks: Foundations - Why documentation sits at the core of quality, compliance, revenue, and outcomes; how HCCs, RAF scores, and annual condition "reset" work. The MEAT framework - Applying Monitor, Evaluate, Assess, and Treat criteria so every reported diagnosis is fully supported. Condition-specific guidance - Detailed documentation and ICD-10-CM capture for morbid obesity and BMI, COPD subtypes and chronic respiratory failure, active versus historical and metastatic cancer, diabetes with complications and remission, CKD staging, depression, and substance use disorders. Annual Wellness Visit - Leveraging the AWV to capture chronic, stable conditions and distinguish "history of" from "chronic, stable." Best practices - Linking conditions, improving diagnosis specificity, and handling uncertain diagnoses compliantly. Knowledge checks - Ten interactive question-and-answer scenarios that let participants apply the concepts and self-assess.

Cancellation Policy
