Physician Documentation Initiative: Provider Education Sessions

This activity delivers interactive education through scheduled 60-minute 1:1 sessions as part of the Physician Documentation Initiative. Sessions are conducted primarily in person, with virtual options available to accommodate clinical schedules. The activity is designed to address practice gaps in clinical documentation that impact Hierarchical Condition Category (HCC) coding, Risk Adjustment Factor (RAF) scoring, and quality performance metrics. Through personalized feedback and real-world case examples, participants will examine their documentation practices and identify opportunities to improve accuracy and specificity. Educational content emphasizes the connection between complete documentation, appropriate risk capture, and the accurate representation of patient complexity. The activity also highlights increasing transparency in provider performance data and its implications for quality reporting, reimbursement, and value-based care. By supporting reflection and application in practice, this activity aims to foster sustainable documentation habits, improve risk adjustment outcomes, and enhance overall quality of care.

1. Apply best practices for accurate and complete clinical documentation to improve representation of patient complexity.
2. Integrate risk adjustment principles, including HCC capture and RAF impact, into routine documentation workflows.
3. Identify and address common documentation gaps that affect quality reporting and risk adjustment.
4. Implement practical tools and strategies to promote sustained documentation improvement within their clinical teams.

Cancellation Policy

Cancellations received in writing no less than 20 days before the course will be refunded, less a 20% administrative fee. No refunds will be made on cancellations received after that date. Please send cancellation requests to [email protected].

Our Lady of the Lake Continuing Medical Education reserves the right to cancel or postpone this program if necessary; in the event of cancellation, course fees will be fully refunded. We are not responsible for other costs incurred such as non-refundable airline tickets or hotel penalties.
